To become an ISR/Affiliate you must first agree to this agreement:
1. I understand that I am NOT employed by Nationwide Wireless. I am only a part of the ISR/Affiliate Program.
2. I understand that I am responsible for my own taxes and I will receive a W-9 form from Nationwide Wireless.
3. I understand my W-9 form must be returned to headquarters before I receive my first commission check.
4.I understand that commissions are paid according to the previous month's performance. Example: All sales made in December will be paid by the 10th of January.
5. I understand that I will be paid via check from Nationwide Wireless.
